License for Veterinary Practice

Veterinary practice is a type of economic activity on diagnosis, preventive treatment, health-improving, clinical work in the field of animal husbandry, veterinary and sanitary expertise on animal and plant products, or on provision of other veterinary services. Veterinary practice may engage persons having relevant education and meeting the qualification requirements.

Qualification requirements are confirmed by:

  • state-recognized degree in veterinary;
  • certificate of professional development (specialists in veterinary medicine are obliged to pass an advanced training at least one time per each 5 years).

License for veterinary practice is a state-recognized document, which gives the right to:

  • produce the and wholesale and retail trade in veterinary medicines and drugs;
  • veterinary practice and allows to implement the specified types of activities in the territory of Ukraine.

  1. Standard application form;
  2. Articles of Association (noarized copy);
  3. Extract from the Unified State Register (noarized copy);
  4. Certificate from the State Statistics Service (noarized copy);
  5. Documents confirming the right to possession and use of or the title for non-residential premises;
  6. Fire Department permit;
  7. State Sanitary and Epidemiological Service permit for premises;
  8. Workplace conformity certificate issued by the municipal Department of Veterinary Medicine;
  9. Certificate of conformity for material and technical facilities;
  10. Notarized copies of the state-recognized degrees in veterinary medicine for the staff and extracts from their work record cards;
  11. Contract with the State Laboratory of Veterinary Medicine for medical tests;
  12. M-2 Form / blank form with the seal on its back-side for a license.
  13. Power of Attorney to act on your behalf.
